• Therapy Changes Lives LLC needs this information for insurance, billing and state licensure scope of practice. Clients contact information is used to schedule and confirm appointments or verify insurance coverage/limits/deductibles.
• Customer information is never shared with third parties/affiliates for marketing/promotional purposes.
